This role is vital in providing high-quality social services to patients and their families, ensuring their needs are met in a supportive environment. Key ResponsibilitiesConduct assessments to determine the social, emotional, and financial needs of patients.Develop and implement individualized care plans in collaboration with healthcare teams.Provide counseling and support to patients and families facing various challenges.Facilitate communication between patients, families, and healthcare providers.Assist in resource allocation and connect patients with community services.Maintain accurate documentation of services provided and patient progress.RequirementsLicensure/Certification:Current California BBS Certification.Basic Life Support (BLS) certification from AHA.Experience: Minimum of 2 years of experience in social work.Active CA Licensure: Acceptable candidates include LCSWs, ACSWs (MSWs), LMFTs, and AMFTs (MFTs) registered within California.Additional InformationSchedule: Will alternate weekly between Tuesday to Friday and Wednesday to Saturday.For Travel Nurse RATE: $60 per hr / $50 for Local *50 mile radius .If you are passionate about providing exceptional social work services and looking for an opportunity to grow in a supportive environment, we encourage you to apply!
