Forest Manor is currently hiring for a full - time RN to work in MDS Office and to assist with RN Assessments. Requires on call coverage 2 to 3 times per month with on call pay.

JOB DESCRIPTION:
The Assessment Nurse (AN) assists in planning, implementing, and evaluating nursing care with the Resident Assessment Instrument (RAI) coordinator. If the AN is a Registered Nurse, the AN may be designated as RAI coordinator. The AN assists with the pre-admission screening process, admission process and nursing interventions. The AN is responsible for clinical documentation to include observing, recording, and reporting to the appropriate person the signs and symptoms that may be indicative of a change in the resident. The AN communicates with the physician, nurses, nurse aides, interdisciplinary team members, residents and resident representatives as to resident’s condition or response to treatment. The AN follows and completes the RAI process including documentation requirements, Minimum Data Set (MDS), Care Area Assessment process, care plan development, and care plan implementation. The Assessment Nurse is responsible to the Director of Nursing.
Education: Must be a graduate of an accredited school of nursing currently registered with the state agency for nursing licensure and hold a valid Nurse’s License in the stat the Assessment Nurse is employed.
Qualifications:
1. At least one year of long term care experience
2. Demonstrates leadership and managerial ability balanced by strong clinical knowledge and skills.
3. Has the ability to organize, plan and supervise the Nursing Department in coordination with other resident services.
4. Demonstrates leadership and management ability to lead the Nursing Department in a cost effective, professional manner
5. Maintains effective interpersonal relationships and works well with the general public.
6. Is capable of implementing changes as mandated by Federal, State and management recommendations.
7. Is of good emotional, mental and physical health, having sound judgement and high profession standards.
8. Has a desire to work with the geriatric clientele as well as residents of any age and their current prospective needs.
9. Must be willing to be in the facility working with the staff of each shift at varying times.
10. Must be willing to be in and/or available to the facility weekends as well as weekdays.
11. Must be willing to participate in the on call schedule.
12. Must demonstrate ability to follow recommendations as designated by the Director of Nursing
13. Must demonstrate the ability to function as a cooperative team member with all disciplines
14. Is willing and capable of providing emergency care as needed for any resident in the facility.
15. Must have a pre-employment and an annual Tuberculosis screening examination as required by the state.
Job Type: Full-time
Pay: $30.00 - $35.00 per hour
