Your Responsibilities as an LPN:
Provide direct nursing care to residents in accordance with their care plans
Administer medications and treatments as prescribed by physicians
Monitor residents for changes in condition and report appropriately
Maintain accurate documentation of care, medications, and observations
Assist with admissions, transfers, and discharges
Communicate effectively with residents, families, and interdisciplinary team members
Promote resident dignity, independence, and overall well-being
Qualifications:
Valid LPN license in the state of Ohio (Required)
Previous experience in long-term care or skilled nursing preferred but not required
Strong clinical and assessment skills
Excellent communication and organizational skills
Compassionate, team-oriented, and dependable
Flexibility to work various shifts, including weekends and holidays
