What are the responsibilities and job description for the Elder Clinical Manager position at Forest County Potawatomi Community?
Here’s what you’ll be doing:
- Supervises and evaluates RNs, LPNs and CNAs in the delivery of care. Delegates medical acts to RN’s, LPN’s and CNA’s in accordance with the Wisconsin Board of Nursing rules on delegation
- Assists with maintaining process, policies, procedures, protocols, etc. to assure care is delivered in an effective, safe, high-quality manner.
- Completes chart audits to assure quality care and quality improvement.
- Coordinates and manages care plan meetings with resident and family members.
- Completes performance appraisals on all nursing staff.
- Reports potential or actual resident safety concerns, medical errors and/or near misses in a timely manner
What you’ll need to be successful:
- Bachelor’s Degree, Associate Degree or Diploma in Nursing. Wisconsin Licensed Registered Nurse required.
- Two years of patient care experience in a medical setting, preferably in long term care
- Strong written and verbal communication skills
- Familiarity with Windows based operating systems and EHR programs.
- Must successfully pass all applicable background checks and drug screens
- FCP complies with all CDC vaccine and testing recommendations for healthcare professionals. Proof of immunizations is required and immunizations/testing provided upon hire.
