Position Summary
We are seeking a dependable, compassionate, and motivated Social Worker / Independent Living Skills Coach to join our team in San Diego. This is a full-time, on-site role providing direct support to adults working toward greater independence in the community.
The ideal candidate enjoys working one-on-one with clients, building trust, solving problems, and helping individuals achieve meaningful goals.
Key Responsibilities
- Provide direct one-on-one support to clients in home and community settings
- Assist clients in developing independent living skills such as:
- budgeting and money management
- meal planning and grocery shopping
- scheduling appointments
- personal organization
- household management
- accessing benefits and community resources
- Transport or accompany clients to appointments, errands, community activities, and other essential services as needed
- Develop and implement individualized service plans based on client goals
- Conduct assessments and monitor client progress
- Maintain accurate documentation, case notes, and reports
- Coordinate care with families, regional centers, healthcare providers, and other service professionals
- Support emotional wellness, confidence-building, and healthy decision-making
- Maintain professional boundaries and uphold client dignity and confidentiality
Qualifications
- Bachelor’s degree in Social Work, Psychology, Human Services, or related field preferred
- Relevant experience working with vulnerable populations, behavioral health, developmental disabilities, or community support services preferred
- Strong communication, listening, and interpersonal skills
- Ability to work independently and manage a flexible schedule
- Professional, dependable, and compassionate approach to client care
- Valid driver’s license with reliable transportation
- Ability and willingness to drive clients to appointments and community activities when needed
- Proof of auto insurance and acceptable driving record preferred
- Master’s degree or licensure is a plus
