Duties
- Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility
- Provide specialized care for patients with dementia, Alzheimer's disease, or other memory impairments, including memory care techniques
- Operate medical equipment such as Hoyer lifts and other mobility aids safely and effectively
- Perform light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe environment
- Document patient progress accurately and communicate updates to healthcare teams
- Work with individuals who have developmental disabilities or disabilities requiring long-term care support
Skills
- Strong patient observation skills to detect changes in condition or behavior
- Knowledge of medical terminology and clinical procedures relevant to home health care
- Experience working in assisted living facilities, nursing homes, or home & community care settings
- Proficiency with EMR/EHR systems for medical documentation and record-keeping
- Ability to assist with ADLs and provide compassionate senior care or hospice support
- Certification in CPR and first aid; additional certifications such as medication administration or catheter care are preferred
- Familiarity with dementia care strategies including Alzheimer’s and memory care techniques
- Experience working with individuals with disabilities or developmental challenges
- Ability to operate mobility aids like Hoyer lifts safely and effectively
- Excellent communication skills for working collaboratively with healthcare teams and families
