Job Overview
We are seeking a compassionate and dedicated Home Health Aid/CNA to provide exceptional in-home care and support to individuals with diverse healthcare needs. Duties

• Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility
• Provide specialized care for patients with dementia, Alzheimer's disease, or other memory impairments, including memory care techniques
• Operate medical equipment such as Hoyer lifts and other mobility aids safely and effectively
• Perform light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe environment
• Document patient progress accurately and communicate updates to healthcare teams
• Work with individuals who have developmental disabilities or disabilities requiring long-term care support

Skills

• Strong patient observation skills to detect changes in condition or behavior
• Knowledge of medical terminology and clinical procedures relevant to home health care
• Experience working in assisted living facilities, nursing homes, or home & community care settings
• Proficiency with EMR/EHR systems for medical documentation and record-keeping
• Ability to assist with ADLs and provide compassionate senior care or hospice support
• Certification in CPR and first aid; additional certifications such as medication administration or catheter care are preferred
• Familiarity with dementia care strategies including Alzheimer’s and memory care techniques
• Experience working with individuals with disabilities or developmental challenges
• Ability to operate mobility aids like Hoyer lifts safely and effectively
• Excellent communication skills for working collaboratively with healthcare teams and families
