Key Responsibilities:
- Perform comprehensive physical assessments and analyze medical histories to evaluate patients' conditions accurately.
- Create and implement tailored treatment plans that address individual rehabilitation goals and patient needs.
- Conduct therapeutic exercises and training programs to improve mobility and enhance overall functionality.
- Educate patients and their families about injury prevention, wellness strategies, and the recovery journey.
- Monitor and document patients' progress, adjusting treatment plans as necessary to achieve rehabilitation goals.

- Currently licensed to practice as a Physical Therapist in the state of agency operation, in full compliance with accepted professional standards and practices
- Minimum of nine months of hands-on clinical experience as a Physical Therapist
- Community or home health experience strongly preferred
Required Certifications & Licensures
- Master's degree in Physical Therapy from a CAPTE-accredited program
- Current CPR certification, maintained continuously throughout employment in a clinical role
- Valid driver's license with the ability to travel across all business locations as needed
