Key Responsibilities:
- Conduct thorough assessments to develop personalized care plans for patients.
- Coordinate and manage patient visits, ensuring alignment with their needs and physician directives.
- Manage medication administration and orders in a timely manner.
- Supervise and support LPNs, LVNs, and Hospice Aides as per established protocols.
- Collaborate with an interdisciplinary team to provide holistic care, addressing physical, emotional, and spiritual needs.
- Educate families and caregivers, providing resources to support them during this challenging time.
- Maintain a compassionate and respectful approach, upholding the values of empathy and integrity in all interactions.
Qualifications:
- Associate's degree preferred; Registered Nurse license required.
- At least one year of nursing experience, with prior hospice experience favored.
- Willingness to participate in on-call rotations.
- Possession of reliable transportation and a valid driver's license.
