We are seeking a dedicated and experienced Home Hemodialysis Training Nurse to join our dynamic healthcare team. Key Responsibilities:
- Conduct individualized training sessions with patients and their caregivers to teach safe and effective home hemodialysis techniques.
- Assess patient readiness for home hemodialysis and develop personalized training plans to address specific needs and learning styles.
- Monitor patient progress and provide ongoing support, education, and encouragement to ensure successful adaptation to home treatment.
- Collaborate with the healthcare team to coordinate care and ensure continuity of services for patients transitioning to home hemodialysis.
- Maintain accurate and detailed records of patient training sessions, progress, and any changes in treatment protocols.
- Participate in quality improvement initiatives to enhance home hemodialysis program effectiveness and patient outcomes.
Qualifications:
- Registered Nurse (RN) with a current state license to practice in Kentucky.
- PD Highly preferred
- Minimum of 2 years of experience in dialysis nursing, with prior experience in home dialysis training preferred.
- Strong clinical assessment and patient education skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a collaborative team.
- Proficient in using electronic medical records and technology relevant to home dialysis.
