What You’ll Be Doing
In this role, you will serve as the clinical anchor of a dedicated care team, overseeing the full continuum of patient health and wellness. Responsibilities include:
- Building and maintaining long-term, trust-based relationships with patients
- Managing chronic and acute conditions using evidence-based practices
- Conducting well visits and occasional house calls
- Providing after-hours phone support for continuity of care
- Monitoring both physical development and psychological well-being
- Leading a care team that includes a Registered Nurse and Patient Care Administrator
- Creating personalized health reports to guide patients and team members toward shared goals
- Participating in regular team huddles to proactively identify and address care needs
Ideal Candidate Profile
We’re looking for a physician who blends clinical rigor with intellectual curiosity and thrives in a collaborative, high-touch environment.
Qualifications
- 5 years of full-time clinical experience, including 3 years in a high-volume setting
- Background in large hospital systems or similarly demanding clinical environments
- Experience working with underserved populations is a plus
- Academic credentials from top-tier U.S. or European institutions across at least two of the following: undergraduate, medical school, residency
