Job Overview
We are seeking a highly skilled and compassionate Nurse Practitioner to join our independent Nurse Practitioner based team. This role involves providing comprehensive primary care services across diverse patient populations, ages 15 , geriatrics, and acute care settings. The ideal candidate will have clinical experience (preferred), proficiency with electronic health record (EHR) systems such as Athena and Epic, and a strong background in various specialties including internal medicine, geriatrics, and behavioral health. The Nurse Practitioner will collaborate with multidisciplinary teams to deliver high-quality patient care, perform diagnostic evaluations, develop treatment plans, and support patient education and health coaching.
Annual Salary with performance based incentives.
Duties
- Conduct thorough physical examinations and patient assessments across outpatient and telehealth settings.
- Manage acute and chronic conditions such as pain management, infectious diseases, geriatrics, and post-acute care.
- Utilize advanced EMR/EHR systems like Athenahealth and Epic for documentation of medical records and utilization review.
- Collaborate with specialists in areas such as cardiology (cardiac catheterization), ophthalmology (ophthalmology), nephrology (dialysis), pulmonology (ventilator management), and infectious disease care.
- Participate in health coaching, and patient education efforts to promote wellness and disease prevention.
- Adhere to HIPAA regulations for patient privacy.
- Support patients with developmental disabilities or memory care needs including Alzheimer’s disease or dementia care.
Requirements
- Valid Nurse Practitioner certification/license with current state registration.
- Proficiency with EMR/EHR systems including Athenahealth and Epic and experience with medical documentation is essential.
- Strong knowledge of physiology, anatomy, pharmacology (including psychopharmacology), medical terminology; skilled in physical examinations and diagnostic evaluation techniques.
- Knowledge of Workers' Compensation law; familiarity with hospice & palliative medicine is a plus.
- Excellent communication skills for triage assessments and patient education across diverse populations including 15 and seniors (geriatrics). Pay: $100,000.00 - $145,000.00 per year
