Are you a compassionate and dedicated Licensed Practical Nurse (LPN) looking for a rewarding opportunity to provide personalized, one-on-one patient care?
We are seeking a qualified LPN to provide skilled nursing care to patients in their homes across Acton, Arlington, Ayer, and Concord, Massachusetts. Daily visits are currently available during both morning and evening hours, offering flexibility to fit your schedule.

- Perform patient assessments, monitor vital signs, and document changes in health status.
- Administer medications and provide skilled nursing treatments according to physician orders and care plans.
- Provide wound care, catheter care, tube feeding support, and other appropriate nursing interventions.
- Support patients with chronic conditions, dementia, developmental disabilities, and age-related health needs.
- Educate patients and families on medications, disease management, and home safety.
- Collaborate with physicians, caregivers, and other healthcare professionals to ensure coordinated care.
- Maintain accurate patient records and follow HIPAA, infection control, and applicable nursing standards.
- Current, valid Massachusetts LPN license in good standing.
- Previous experience in home health, skilled nursing, long-term care, or a related clinical setting preferred.
- Strong clinical assessment, medication administration, and patient care skills.
- Experience working with older adults, individuals with developmental disabilities, or patients with chronic conditions is a plus.
- Familiarity with electronic medical records (EMR/EHR) and clinical documentation.
- Excellent communication, organization, and time-management skills.
