As a growing provider in Sacramento, this Registered Nurse role is an excellent opportunity to get in early with a trusted, established organization. While we are currently hiring for per diem and part-time, this position is expected to grow into a full-time role as our patient census and services expand in the region.

What You'll Do
- Perform comprehensive in-home nursing assessments for home health patients
- Develop and manage individualized plans of care in collaboration with physicians, caregivers, and the interdisciplinary care team
- Provide skilled nursing services including medication management, wound care, disease management, and patient education
- Complete OASIS assessments, recertifications, case conferences, and timely clinical documentation
- Identify the need for additional home health services, disciplines, and durable medical equipment (DME)
- Assess home environments to promote patient safety, fall prevention, and improved outcomes
- Obtain and document physician orders, including verbal orders, per agency and regulatory guidelines
- Educate patients and caregivers on care plans, medications, and symptom management
- Maintain compliance with CMS, Medicare, and home health regulations
- Serve as a preceptor or mentor to other nursing staff as needed

- Graduate of an accredited nursing program
- Current, active California Registered Nurse (RN) license and CPR certification
- At least one year of clinical practical nursing experience outside of school or rotations
- Home Health experience preferred, but not required
- Valid driver's license, reliable vehicle, and current auto insurance
- Strong interpersonal and communication skills
- Ability to make home visits and provide skilled nursing assessments and care
- Home Health and Home Care Home Base (HCHB) documentation experience preferred, but not required
