Responsibilities:
- Provide direct patient care, including bathing, dressing, grooming, and toileting, under the supervision of licensed nursing staff
- Assist patients with activities of daily living (ADLs), such as eating, drinking, and ambulating
- Monitor patients' vital signs and report any changes or concerns to licensed nursing staff
- Maintain accurate and up-to-date records of patient care and observations
- Collaborate with interdisciplinary teams to develop and implement individualized care plans
- Participate in care conferences and meetings to discuss patient progress and goals
- Perform other duties as assigned by licensed nursing staff
- Current certification as a Certified Nursing Assistant (CNA) in the State of Pennsylvania
- High school diploma or equivalent required
