Description:

Position Overview: The Nurse Practitioner – Adult/Gerontology (AGNP/GNP) provides advanced nursing care for older adult patients across clinic and nursing home settings. The AGNP/GNP assesses, diagnoses, and manages acute and chronic conditions, implements evidence-based treatment plans, and provides preventive care tailored to the geriatric population. This role includes direct patient care in clinic visits and scheduled nursing home rounds, ensuring continuity of care, patient safety, and adherence to HIPAA and organizational policies.

The AGNP/GNP collaborates with physicians, nursing staff, therapists, social workers, and other interdisciplinary team members to optimize functional outcomes, manage comorbidities, and support quality of life for older adults.

Duties and Responsibilities:

  • Conducts comprehensive assessments of geriatric patients, including physical exams, functional evaluations, and review of medical history.
  • Provides diagnosis, treatment, and management of acute and chronic conditions in geriatric patients.
  • Develops, implements, and adjusts individualized care.
  • Orders, performs, and interprets diagnostic and laboratory studies as appropriate.
  • Prescribes medications, non-pharmacologic therapies, and other treatments within scope of practice and regulatory guidelines.
  • Educates patients and families on medications, treatments, disease prevention, and health promotion, with consideration for cognitive or functional limitations.
  • Conducts scheduled nursing home rounds, coordinating with facility staff to review patient status, monitor outcomes, and recommend care adjustments.
  • Provide on-call coverage for nursing home patients as scheduled, including triage, care coordination, and consultation with the patient’s primary physician as needed.
  • Documents all care and interactions accurately and in a timely manner in compliance with HIPAA and Medical Center policies.
  • Initiates referrals, consultations, and therapeutic interventions; collaborates with specialists to coordinate care plans.
  • Collaborates with interdisciplinary teams to optimize patient outcomes and maintain quality standards of care.
  • Monitors patient safety and demonstrates situational awareness in all care environments.
  • Complies with departmental, Medical Center, and regulatory guidelines, including American Nurses Credentialing Center (ANCC) and American Academy of Nurse Practitioners (AANP) Code of Ethics and Standards of Practice.
  • Participates in quality improvement initiatives focused on geriatric care.
  • Maintains professional competence through continuing education and professional organization involvement.
  • Meets or exceeds RHC productivity standards.

Minimum Education and/or Experience Required:

  • Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) Degree required.
  • Two years professional nursing experience preferred; experience in geriatric care or primary care for older adults strongly preferred.
  • AGNP (Adult-Gerontology Nurse Practitioner) credential preferred; may be waived for extensive geriatric experience.

Licensure/Certification Required:

  • Current Iowa Registered Nurse (RN) license.
  • Current Iowa Advanced Registered Nurse Practitioner (ARNP) license.
  • Adult-Gerontology Nurse Practitioner (AGNP) credential preferred; may be waived for extensive geriatric experience.
  • Drug Enforcement Administration (DEA) Certificate.
  • Controlled Substances Act (CSA) Certificate.
  • Basic Life Support (BLS) Certification. (BLS is not required for staff with Advanced Cardiac Life Support (ACLS) and Pediatric Advanced Life Support (PALS) certifications).
  • Advanced Cardiac Life Support (ACLS) Certification preferred.
