Job Summary
Join our dedicated home care team as a Per Diem Physical Therapist, where you will play a vital role in delivering personalized rehabilitation services directly in patients’ homes. You will assess patient needs, develop tailored care plans, and provide expert physiotherapy interventions to promote optimal mobility and function across diverse populations, including pediatrics, geriatrics, and individuals recovering from acute or chronic conditions.
Duties
- Conduct comprehensive patient evaluations to determine functional status and physiotherapy needs within the home environment.
- Develop and implement individualized care plans focused on improving mobility, strength, and overall well-being.
- Deliver targeted physiotherapy treatments such as trigger point therapy, orthopedics-focused exercises, and gait training to facilitate recovery.
- Coordinate discharge planning with healthcare teams to ensure seamless transition from hospital or inpatient settings to home care.
- Monitor patient progress regularly, adjusting care plans as needed to meet evolving goals.
- Document all patient interactions accurately using electronic medical record (EMR) systems, maintaining detailed records of treatment plans, progress notes, and outcomes.
- Collaborate with physicians, caregivers, and other healthcare professionals to optimize patient care across outpatient and community settings.
Requirements
- Valid state licensure as a licensed Physical Therapist with current registration.
- Proven experience in inpatient hospital settings, outpatient clinics, or home health environments.
- Strong knowledge of physiology, anatomy, medical terminology, and care plan development.
- Demonstrated expertise in discharge planning, acute care protocols, and community-based physiotherapy practices
- Proficiency with EMR systems for documentation and communication purposes.
- Excellent patient care skills with a compassionate approach tailored to individual needs in home or community settings.
- Ability to work independently while managing multiple cases efficiently; previous home & community care experience is highly valued. Pay: $75.00 - $100.00 per hour
