Assistant Social Worker Part Time
Purpose of this position
Assess and provide social services primarily to the short and long term residents of Porter McGrath Health Center as well as serve as a resource to the greater FPH community
Position Requirements
Educational:
- Bachelor of Social Work (BSW) degree preferred, but not required
a quick study is needed.
Experience:
- Social work field placement or case management in health care , with experience in long term care preferred
- Interest in Geriatrics and Life Care Community
- Excellent communication and problem-solving skills
- Computer proficiency in health care systems and MS Office
- Must have the ability to function independently and as a team player.
- Ability to work in a demanding, fast-paced environment
Specific Job Functions
Technical Functions:
- Assesses and re-assesses residents for psychosocial and discharge planning needs.
- Completes Social Service MDS (Minimum Data Sets)according to requirements and develops suitable care plans as an interdisciplinary team member
- Assists with monthly Resident Council Meetings, typing minutes and attending GDR meetings when SSD is out
- Uses knowledge of resources and services to coordinate discharge arrangements or services in response to identified needs, and according to policies and applicable State and Federal regulations
- Coordinates with patients, families and staff to arrange care planning meetings, and acts as an active participant.
- Ensures review and/or completion of required forms, including admission and discharge materials
- Monitors compliance by completing LOC through Elder Affairs and performing NOMNC and Advanced Directive Audits regularly
- Supports residents with a variety of social services such as coordination of dental, vision, and podiatry services, personal funds accounts, personal safe access, agency referrals, resources for visually or hearing impaired, and mail distribution.
- Participates in Quality Improvement activities and reporting
- Functions as a resident advocate follow protocols in assisting with room changes and the investigation and required reporting of grievances and allegations of abuse.
Pay Range:
$15 to $21
