Florentine Gardens is seeking a compassionate and detail-oriented Registered Dietitian to join our interdisciplinary care team. The Dietitian is responsible for assessing residents’ nutritional needs, developing individualized care plans, and ensuring compliance with state and federal regulations for long-term care facilities.
Key Responsibilities
- Complete comprehensive nutrition assessments for new admissions, quarterly reviews, and significant changes
- Develop, implement, and monitor individualized nutrition care plans
- Collaborate with nursing, dietary, therapy, and medical staff to ensure coordinated resident care
- Review and approve menus to ensure compliance with physician orders and regulatory standards
- Monitor residents for weight changes, wounds, swallowing concerns, and therapeutic diet compliance
- Provide nutrition education and counseling to residents, families, and staff as needed
- Participate in care plan meetings and Quality Assurance (QAPI) initiatives
- Ensure compliance with CMS, state, and local health department regulations
- Oversee documentation related to nutritional services
Qualifications
- Registered Dietitian (RD) credential required
- Licensed Dietitian in the State of Ohio (or eligible)
- Experience in long-term care or skilled nursing preferred
- Knowledge of CMS guidelines and survey process
- Strong communication and organizational skills
- Ability to work independently and as part of a team
