Job Summary
We are seeking a compassionate and dedicated Certified Home Health Aide (CHHA) to provide high-quality patient care in . Responsibilities
- Provide personal care assistance, including helping with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support.
- Monitor and observe patients' conditions, reporting any changes or concerns promptly.
- Assist with medication administration following prescribed protocols and maintaining accurate medical documentation.
- Utilize Electronic Health Record (EHR) or EMR systems to document patient care activities accurately and securely in compliance with HIPAA regulations.
- Support patients with meal preparation, feeding, laundry, light cleaning, and maintaining a safe environment.
- Assist with mobility using equipment such as Hoyer lifts and help patients transfer safely.
- Take vital signs including blood pressure, pulse, respiration, and temperature to monitor health status.
- Support patients with cognitive impairments such as Alzheimer's disease or dementia through memory care techniques and behavioral management strategies.
- Assist with medical procedures like catheter care or hospice-related tasks as needed.
- Work collaboratively with healthcare teams to ensure comprehensive patient care in home or community settings.
Experience
- Prior experience in nursing homes, assisted living facilities, or home health environments preferred.
- Experience working with people with developmental disabilities or disabilities requiring long-term care is highly desirable.
- Knowledge of medical terminology, medication administration protocols, and medical documentation practices.
- Certifications such as CPR are required; additional certifications related to Alzheimer's care, hospice care, or dementia support are a plus.
- Familiarity with EMR/EHR systems and HIPAA compliance is essential.
- Previous caregiving experience involving patient observation, assisting with ADLs, and providing compassionate senior or home care is highly valued.
