Job Overview
We are seeking compassionate and dedicated Certified Home Health Aide (CHHA) professionals to provide high-quality patient care in
Duties
- Provide personalized patient care, assisting with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support
- Monitor and record vital signs accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems
- Assist with medication administration following proper protocols and documentation standards in compliance with HIPAA regulations
- Support patients with dementia, Alzheimer's, or other memory-related conditions through specialized memory care techniques
- Operate medical equipment such as Hoyer lifts, catheters, and other assistive devices safely and effectively
- Assist with meal preparation and ensure proper nutrition for patients based on dietary needs
- Perform light housekeeping duties including laundry and cleaning to maintain a safe and hygienic environment
- Document patient progress and communicate effectively with healthcare teams regarding changes in condition or care plans
- Support hospice care patients by providing compassionate end-of-life assistance in accordance with care plans
Qualifications
- Valid certification as a Certified Home Health Aide (CHHA) or equivalent credential required
- Prior experience in nursing homes, assisted living facilities, home health care, or working with people with developmental disabilities preferred
- Knowledge of medical terminology and medical documentation practices
- CPR certification required; additional certifications such as medication administration, catheter care, hospice care, or dementia care are a plus
- Experience working with EMR/EHR systems and understanding of HIPAA regulations for patient privacy
- Ability to work independently and compassionately with diverse patient populations including seniors and individuals with disabilities
- Strong communication skills and the ability to work well within a team environment
- Flexibility to work live-in shifts or extended hours as needed
