Job Overview
We are seeking a compassionate and dedicated caregiver to provide high-quality patient care in a home or community setting. The ideal candidate must be able to drive to work locations reliably and demonstrate a strong commitment to supporting individuals with diverse needs, including seniors, those with disabilities, or individuals requiring specialized medical assistance. This role involves assisting with daily activities, ensuring safety, and maintaining accurate medical documentation to support overall well-being.
Responsibilities
- Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support
- Monitor and observe patients’ conditions, including vital signs and behavioral changes
- Administer medications and document medication administration accurately following HIPAA guidelines
- Support patients with dementia, Alzheimer’s, or memory care needs through specialized care techniques
- Operate Electronic Health Record (EHR) or EMR systems for accurate record-keeping
- Assist with medical procedures such as catheter care, Hoyer lifts, and hospice care as needed
- Prepare meals and assist with meal planning in accordance with dietary restrictions
- Perform light housekeeping tasks including cleaning and laundry to maintain a safe environment
- Ensure patient safety by adhering to care plans and safety protocols
- Communicate effectively with healthcare teams regarding patient status and progress
Qualifications
- Valid driver’s license with reliable transportation to work locations
- Prior experience in assisted living, nursing homes, or home health care preferred
- Knowledge of medical terminology and documentation standards such as HIPAA
- Experience working with individuals with developmental disabilities, dementia, Alzheimer’s, or memory care needs is highly desirable
- Certifications such as CPR are preferred but not mandatory; willingness to obtain required certifications is expected
- Ability to work independently and compassionately in a home or community setting
- Strong observational skills for patient monitoring and safety assurance
- Excellent communication skills for effective interaction with patients, families, and healthcare teams
