What are the responsibilities and job description for the PRN Mental Health Counselor position at FLINT RIVER HOSPITAL?
Position Summary:Will be the liaison between the various units, psychiatrists, patients and families. He/She will provide an early assessment; coordinate with the treatment team, patient/family and community resources and provide group therapy sessions. Will also develop and assist with implementation of the patient’s treatment plan and offer support education to patients and families as needed. Will work with the team in formulating a viable discharge plan.
Essential Duties and Responsibilities:
The essential functions include, but are not limited to the following:
- Observes professional ethics in maintaining confidential information acquired concerning all information of patients in the units
- Provides patients with a bio-psychosocial assessment and written discharge plan that is appropriate for the patients’ needs and coordinates patient’s discharge
- Coordinates patient/family participation in treatment process
- Provides services appropriate to the age and functional status of the patients during group times and throughout patient’s day
- Provides psychosocial/therapeutic intervention to patients to maintain or improve patient’s physical, mental and psychosocial needs
- Performs pre-assessment screenings as needed and coordinates patient’s transition from community to unit
- Assist with community education efforts and communication with referral sources as needed
- Provides crisis intervention services, as needed, completion of this function may extend past usual work hours
- Assist with on-call duties as needed
- Assist with patient care as needed (may include, but not limited to, setting up meal trays, sitting in the day room, sitting 1:1 with patients, etc.
- Assumes additional duties as assigned by program director
Minimum Qualifications (Knowledge, Skills and Abilities)
Bachelor's degree from an accredited program in Social Work, Psychology, or related mental health discipline
Working knowledge of community services and resources
Demonstrated leadership
Superior communication and interpersonal skills
General knowledge of psychiatric care and needs
Ability to perform and proceed on his /her own initiative in compliance with policies, practices and procedures of the facility and program director
Qualifications Preferred
Experience in Mental Health Counseling in an acute hospital setting
Working knowledge in formulating a Plan of Care
