Responsibilities
- Conduct comprehensive assessments to evaluate patient needs, including physiology, anatomy, and medical history
- Develop and implement effective treatment plans tailored to individual patient goals and conditions
- Assist in discharge planning by coordinating care transitions and providing home & community care guidance
- Deliver hands-on therapy techniques such as trigger point therapy and physiotherapy modalities to facilitate recovery
- Maintain accurate clinical documentation in accordance with healthcare standards and electronic health records (EHR/EMR systems)
- Collaborate closely with physicians, nurses, occupational therapists, and other healthcare providers to optimize patient outcomes
- Educate patients and their families on treatment exercises, mobility strategies, and self-care practices for ongoing rehabilitation
Requirements
- Valid license or certification as a licensed Physical Therapist or PTA in the applicable state or region
- Proven experience practicing in a physical therapy role within outpatient clinics, inpatient hospitals, or home health settings
- Strong knowledge of physiology, anatomy, medical terminology, and neurological conditions
- Experience with discharge planning, care plan development, and clinical documentation standards
- Familiarity with outpatient rehabilitation center operations and inpatient hospital workflows
- Ability to work effectively with geriatric patients, pediatric populations, and those requiring orthopedics or neurology-focused therapies
- Proficiency in electronic health records (EHR/EMR systems), outpatient experience preferred
- Excellent client communication skills to engage patients actively in their rehabilitation journey
