Job Summary: In accordance with FLACRA's mission, vision, and values, the Behavioral Health Community Services Coordinator is responsible for coordinating community-based behavioral health services, including referral management, intake coordination, service assignment, data tracking, reporting, and communication with referral sources. The Coordinator also maintains a caseload and provides rehabilitative and recovery-oriented services in accordance with New York State requirements, managed care guidelines, and agency policies.

Services may include those authorized through Health Home, HARP, HCBS, CORE, or other community-based behavioral health programs as applicable.

Duties

  • Coordinate referrals into community-based behavioral health services, ensuring timely communication with referral sources, care managers, and internal staff.
  • Monitor referral status, authorizations, service thresholds, and client engagement to promote timely access to services.
  • Assign referrals and coordinate intake activities in collaboration with program leadership.
  • Maintain program data, tracking systems, reports, and performance metrics as required by the agency and funding sources.
  • Maintain an assigned caseload and provide community-based behavioral health services in accordance with approved service authorizations.
  • Complete comprehensive assessments, service plans, crisis plans, care plan updates, discharge planning, and other required documentation.
  • Collaborate with Health Home Care Managers, managed care organizations, treatment providers, and community partners to coordinate client care.
  • Participate as an active member of interdisciplinary treatment teams, including treatment planning and case conferences.
  • Provide rehabilitation, recovery, skill-building, advocacy, and supportive services that promote client independence and recovery.
  • Conduct community-based visits, including home visits and other approved community locations, as clinically appropriate.
  • Assist clients in accessing medical, behavioral health, vocational, educational, housing, and other community resources.
  • Provide transportation or transportation coordination when appropriate and consistent with agency policy.
  • Respond appropriately to crisis situations and follow agency protocols to ensure client safety.
  • Maintain compliance with all documentation standards, Medicaid requirements, managed care requirements, agency policies, and applicable federal and state regulations.
  • Participate in staff meetings, supervision, required trainings, and quality improvement activities.
  • Assist with onboarding and training new staff assigned to community-based behavioral health services, as requested.
  • Maintain current knowledge of New York State behavioral health regulations, HCBS, CORE, HARP, Medicaid, and other applicable program requirements.
  • Demonstrate professionalism, ethical practice, cultural competency, and person-centered care consistent with FLACRA's mission and values.
  • Perform other duties as assigned.

Minimum Qualifications: High school diploma or equivalent required; Associate's or Bachelor's degree in Human Services, Social Work, Psychology, Behavioral Health, or a related field preferred. One to three years of experience working with individuals with substance use disorders, serious mental illness, or co-occurring disorders required. Certified Recovery Peer Advocate (CRPA), Certified Peer Specialist (CPS), CASAC, or other credential appropriate to the services provided preferred, as applicable. Experience coordinating referrals, managing caseloads, and working within interdisciplinary treatment teams preferred. Valid New York State Driver's License with an acceptable driving record required.

  • Knowledge of behavioral health, substance use disorder, recovery-oriented services, and community resources.
  • Ability to establish and maintain effective working relationships with clients, families, referral sources, and community partners.
  • Experience using electronic health records (eRecord or equivalent) and Microsoft Office applications.
  • Ability to work independently while collaborating effectively within a multidisciplinary team.
  • Ability to remain calm and exercise sound judgment during crisis situations.
  • Ability to travel throughout the service area to provide community-based services.
