The Practice Administrator for HBPC leads the operational management of our startup home-based primary care practice and its integration with our other healthcare at home services. This role is central to ensuring seamless care delivery to seniors with chronic conditions in the comfort of their homes. The ideal candidate will bring a blend of healthcare operations expertise, leadership and a passion for improving lives through accessible, patient-centered care. The company succeeds when we engage referral partners with our HBPC-infused disease management solutions to reduce institutional costs for payors, reduce length-of-stay for hospitals and SNFs, ensure accountability with patient/family and simplify home retirement.
