The Hire Grant Case Manager reports to the HIRE Program Manager and is accountable for delivering high-quality, cognitive-behavioral group interventions and intensive case management services to individuals upon their release from incarceration in Los Angeles County, including those on community supervision. This Case Manager position is a part of the HIRE Grant Program, a collaboration among Multiple Agencies and Five Keys.

Responsibilities
JOB DUTIES AND RESPONSIBILITIES
- Manage Transitional Coordinators and ensure they are performing their duties
- Attend transitional employment meetings to provide updates and status of current transitions and assigned projects
- Provide documentation of transitions and monthly reports as requested
- Support team with occasional change of hours and duties in order to fulfill contractual obligations
- Approval of punch change requests and time cards in Paycom for all Transitional Coordinators
- Upon participant program entrance, completion, and exit, administer assessments and surveys to participant, including needs Assessments, O*NET
- Maintain case files for all participants that include enrollment and assessment information, Individual Career Plans (ICP), and records of attendance and completion of work-readiness classes
- Coordinate and/or facilitate work-readiness classes that includes the development of employment soft skills, interview preparation, and financial and digital literacy, maintaining accurate records of individual attendance and progress
- Maintain accurate activity records for Five Keys databases and prepare data reports when needed
- Communicate participant employment assessment results, employment plans, and progress with Five Keys staff and partners
- Help to advise participants on career paths and opportunities for placement into jobs, training programs, or college upon transition
- Develop and maintain strong connections and referrals to Job Centers, including the Comprehensive and Reentry Job Centers to ensure continuation of services upon transition
- Work closely with Cal Crew Program Manager and field supervisors assisting the participants with obtaining gainful employment and entering back into the workforce
- Carry out duties and responsibilities in strict compliance with the terms and conditions required by Caltrans in the agreement between BCOE, Caltrans and Five Keys Schools and Programs
- Assist Program Manager with recruiting, hiring and onboarding for transitional employment Back 2 Work Program
- Obtain and Strengthen partnerships with service providers in Los Angeles County and form relationships within the community to enhance the ability of providing additional supportive services needed for participants
- Attend transitional employment meetings to provide updates and status of current transitions and assigned projects
- Provide documentation of transitions and monthly reports as requested
- Support team with occasional change of hours and duties in order to fulfill contractual obligations
- Other duties as assigned
- Manage caseload and maintain regular ongoing contact with participants
- Offer weekly drop in office hours for participants
- Provide participants with one on one, wrap-around employment case management
- Provide participants with, secondary and post-secondary educational support
- Formulate and document a case plan with each participant that identifies core needs and provides appropriate linkages and referrals to services
- Utilize a cognitive-behavioral approach to develop and achieve case plan goals
- Monitor and accurately document the participant’s progress in areas identified in their case plan
- Support participants through the enrollment process in Five Keys schools as needed, and facilitate meetings with assigned teachers
- Support participants through job placement, barrier removal, referral to expungement, document retrieval, and referral to substance abuse treatment and other services
- Ensure information is disseminated to internal and external stakeholders in a timely manner
- Collaborate with program manager, partnering agencies, and other supportive agencies
- Assist with securing a re-entry case plan for each program participant
- Enter case plan and the services provided in Apricot
- Develop transition and/or intervention plans by conducting and analyzing assessments, developing family and community collaborative support teams, and providing service referrals and follow-up
- Attend, participate in, and complete all required program training classes and continuing professional development classes
- Other duties as assigned
KNOWLEDGE AND SKILLS
- Ability to take direction and work independently
- Knowledge of vocational guidance and social adjustment principles, including interviewing and counseling techniques and the social and psychological problems facing offenders
- Knowledge of principles of personality development and adjustment and the social structure of individual assimilation into an offender population
- Ability to maintain strong and appropriate boundaries with personal identifying information when interacting with students, members of the community, or program partners
- Proficient in Zoom, Microsoft Office, Google Workspace, and databases
- Able to maintain strict confidentiality and take all precautions when handling sensitive information
- Exceptional written and verbal communication skills with the ability to express complex ideas with clarity and precision in written documents
- Excellent interpersonal skills
- Excellent attention to detail, organization, and task completion skills
- Advanced problem-solving skills and the ability to tactfully address and resolve questions or concerns
EDUCATION AND WORK EXPERIENCE
- High School Diploma/GED required
- Bachelor’s degree in related field; behavioral or social sciences, criminal justice administration, psychology, or criminology preferred
- 1 year case management / social work experience required
- Previous records management or social casework experience preferred
PHYSICAL REQUIREMENTS

- Must wear provided PPE (Personal Protective Equipment) and adhere to all safety guidelines and requirements, when needed
- Frequent talking and hearing conversations
- Long periods of walking, sitting at a desk and/or computer station
- Lifting items that weigh up to 25 lbs.; reaching, bending, carrying and stooping within the daily routine of tasks and activities
This position is contingent upon:
- Satisfactory results of a Live Scan (criminal background investigation)
- Successfully obtaining jail clearance for positions working in county jails or probation locations
- Submitting proof of clear TB results. Results dated within 60 days prior to the hire date are acceptable
- Moderate to the high noise level
- Work environment varies from site to site
- Must be willing and able to work in correctional facilities or community sites
- Some personal safety risk working in underserved communities
