What are the responsibilities and job description for the Social Worker/Case Manager position at Five Horizons Health Services?
Position Purposes: The Social Worker/Case Manager is responsible for the daily operation and execution of social services as well as coordinating and collaborating with other departments to ensure successful client engagement and social service delivery. The Social Worker/Case Manager works as part of an interdisciplinary health team to coordinate linkage to care, identify and maximize barriers to care and facilitate the ongoing delivery of services to clients of Five Horizons Health Services (FHHS)
Essential Functions:
- Provide direct social work services to a caseload of clients at different levels of care as dictated by their care plans to facilitate client retention in medical care
- Complete biannual psychosocial assessments and case plans with measurable goals and objectives
- Collaborate with client’s medical team to identify and address medical needs related to accessing care and treatment
- Participate in multidisciplinary health team meetings and provide feedback from the social work perspective
- Ensure client’s insurance recertifications are completed in a timely manner according to The Mississippi Department of Health
- Maintain a strong knowledge of available services to include Ryan White HIV/AIDS Program (RWHAP) Part B, and Mississippi Department of Public Health (MSDH)
- Demonstrate a skilled knowledge of engagement, assessment, case planning, and monitoring techniques to maintain the helping relationship between the client and FHHS
- Comply with, and adhere to, organizational and departmental policy as well as the guidelines and standards per funding sources
- Promote client participation in healthcare continuum through face-to-face encounters, and any other engagement techniques as needed
- Enhance clients’ understanding of HIV by demonstrating and relaying a skilled knowledge of HIV / AIDS, Hepatitis C, Pre-Exposure Prophylaxis (PrEP) and their effects on individuals and families
- Maintain required client records, document services provided in a timely manner as required by funding sources, ensure all client documentation has been entered into the EMR system
- Maintain compliance with HIPAA and state guidelines as well as FHHS policy and procedures
- Submit all required data and reports by its respective due dates and accordance with guidelines and policies that have been set forth
- Maintain relationships with community partners and stakeholders
- Perform any other department / agency-related duties or special projects as directed by supervisor
Education/Professional:
- Bachelor’s or Master’s Degree in Social Work (Preferred)
- Human Service Degree (Required)
- Licensure or license-eligible at the appropriate level through the Mississippi State Board of Social Work Examiners
- Must possess a strong ability to interact with diverse groups and cultures
- Strong organizational, time management and effective written and oral communication skills
Physical Requirements: This position requires an individual who is self-motivated and able to independently complete assigned responsibilities. Moreover, this individual must also be able to accept proper supervision and work cooperatively with clients, peers, and other professionals Frequent sitting, some standing / walking, and occasionally lifting supplies / equipment up to 50 lbs. Occasional reaching, bending, stooping, kneeling and crouching. This position requires an individual who is mentally alert and able to effectively communicate verbally and in writing. Individuals must be able to effectively utilize Microsoft Office programming and, after necessary training, electronic health records system and Telemedicine equipment.
