SALARY RANGE: $30,000 - $40,000 DOE
GENERAL SUMMARY:
Will link eligible clients to appropriate, comprehensive, and timely services within the state funded Homecare and Title III Home Delivered Meals Program.
GENERAL DUTIES AND RESPONSIBILITIES:
- Provide an initial assessment and annual reassessment to determine eligibility for the Homecare and Title III Home delivered meal program(s).
- Coordinate services and support from all agencies involved in providing services required by the Plan of Care.
- Maintain client case files that include any client interactions.
- Ensure that all service providers have a working knowledge of the Plan of Care.
- Possess and understanding of federal, state, and internal standard operating procedures, regulations, and general policies and procedures to ensure compliance.
- Assist clients in applying for other services or benefits for which they qualify.
- Responsible for coordinating, arranging, and documenting client services provided by any funding source, volunteer, or natural resource.
- Make a reasonable effort to secure and utilize informal supports for clients and document reasonable effort in the case record.
- Monitor clients by conducting a home visit according to the assessed level of need and make telephone calls between home visits.
- Enter all data into the reporting system in a timely manner.
- Document in the case records each contact made with a client or on behalf of a client.
- Explain necessary paperwork and assist with client complaints as needed.
- Determine fee paying status for clients.
- Travel to required meetings and training courses, including overnight.
- Perform any other duties as assigned.
MINIMUM QUALIFICATIONS:
Must meet on of the following qualifications:
- Possess a minimum of a bachelor’s degree in at least one (1) of the following:
- Social Work.
- Gerontology.
- Psychology.
- Sociology; or.
- A field related to geriatrics.
- Possess a bachelor’s degree in nursing with a current Kentucky nursing license.
- Possess:
- A bachelor’s degree in a field not related to geriatrics with two (2) years of experience working with the elderly; or
- A master’s degree in a human services field, which shall substitute for the required experience.
- Possess an associate degree in a health or family services field and two (2) years of experience working with the elderly, which shall substitute for a bachelor’s degree.
- Be a Kentucky-registered nurse with a current Kentucky license and two (2) years of experience working with the elderly; or
- Be a licensed practical nurse with a current Kentucky license and three (3) years of experience working with the elderly.
SPECIALIZED SKILLS, KNOWLEDGE, AND ABILITIES:
- Knowledge of federal and state statues, regulations, and policies relating to aging and disability issues.
- Knowledge of community resources.
- Skills in computers and detailed reporting.
- Strong interpersonal skills relating to the elderly and disabled.
- Ability to formulate comprehensive planning and to communicate clearly and concisely, both orally and in writing.
- Ability to establish and maintain effective working relationships with co-workers, clients, family members and professional staff.
- Ability to prioritize work, meet deadlines, schedule and keep appointments.
- Ability to keep accurate records and client charts.
- Ability to maintain composure under stressful situations.
- Ability to work independently but under the supervision of the Social Service Director.
- Special licensing requirements must be maintained including a valid driver’s license.
- Frequent public and private contacts requiring tact and diplomacy are required.
- Regular use of confidential information is required.
WORKING CONDITIONS:
- Work is performed indoors and outdoors, regardless of weather.
- Work is performed in a noisy place and can require being in confined spaces or using stairs or ladders.
EFFORT:
- Must be able to see, hear, and speak.
- Work is typically performed while sitting at a desk or table with intermittent standing, stooping, and walking, driving, and all aspects of home visits.
- Lifting light objects (25 lbs. or less) is a job requirement.
- Mental effort is moderate to heavy, and interruptions are frequent to constant.
MACHINES, TOOLS, AND EQUIPMENT:
Normal office equipment will be used (computer, laptop, copier, telephone, fax machine, etc.) and must operate a motor vehicle as a job requirement. The use of Microsoft 365 products is a must.

This organization shall be known as the FIVCO Area Development District, a local agency created under the Kentucky Revised Statutes (KRS Chapter, 147A), with its purpose being to promote, develop and protect the economy, health, education, and general welfare and to implement projects in accordance with KRS 147A for the people of Boyd, Carter, Elliott, Greenup and Lawrence Counties in Northeastern Kentucky, and to further act in conjunction with adjoining Area Development Districts to implement programs of benefit to the Northeastern Kentucky area.The FIVCO Area Development District was founded in 1968 and proudly serves its five-county region in many different capacities. The Board of Directors consists of 25 members from the five-county region.
