What are the responsibilities and job description for the Patient Care Liaison position at Fitchburg Healthcare Center?
Now Hiring a Patient Care Liaison in Fitchburg, MA
Position Summary
The Patient Care Liaison is responsible for supporting census growth by managing the full referral and admission process while developing strong relationships with patients, families, hospitals, physicians, discharge planners, case managers, and other healthcare partners.
This is a highly visible, hands-on role that combines referral management, admission coordination, patient and family engagement, and face-to-face community outreach. The Patient Care Liaison serves as a primary point of contact from the time a referral is received through admission while also actively developing relationships and generating new referral opportunities within the market.
Success in this role requires someone who is equally comfortable reviewing a referral, coordinating an admission, meeting with a family, conducting a facility tour, and spending time in the community building relationships with referral partners.
Essential Duties and Responsibilities
- Referral & Admission Management
- Relationship Development & Community Outreach
- Census & Business Development
- Documentation & Accountability
Key Performance Expectations
Performance will be measured through a combination of referral response time, referral follow-up, admission conversion, census growth, skilled and Medicare/managed care growth, face-to-face outreach activity, referral-source development, CRM documentation, and overall contribution to facility goals.
The Patient Care Liaison is expected to balance both sides of the position. Referral management and admission responsibilities should not eliminate consistent field presence, and external outreach responsibilities should not interfere with timely referral response and admission coordination.
Qualifications
- Previous experience in skilled nursing, post-acute care, healthcare admissions, healthcare outreach, physician relations, business development, or a related healthcare setting preferred.
- Knowledge of the skilled nursing referral and admission process preferred.
- Understanding of Medicare, Medicare Advantage, Medicaid, managed care, and other healthcare payer sources preferred.
- Strong communication, relationship-building, organizational, and follow-through skills.
- Ability to develop professional relationships with healthcare providers, patients, families, and community partners.
- Ability to manage multiple referrals and priorities in a fast-paced environment.
- Comfortable conducting face-to-face outreach, facility tours, patient/family meetings, and professional presentations.
- Working knowledge of electronic referral platforms, CRM systems, and electronic health records preferred.
- Ability to travel throughout the local market as necessary.
- Must demonstrate professionalism, accountability, urgency, compassion, and a strong commitment to customer service.
Ideal Candidate
The ideal Patient Care Liaison is relationship-driven, organized, responsive, competitive, compassionate, and results-oriented. This individual understands that growing a skilled nursing facility requires more than responding to referrals—it requires building trust in the community, creating new opportunities, following every appropriate opportunity through the admission process, and providing patients and families with an exceptional experience from first contact through arrival at the facility.
