What You’ll Do
- Provide skilled, in-home nursing visits (assessment, care planning, teaching, coordination).
- Perform clinical interventions within scope (e.g., medication management/education, disease management coaching, wound and skin assessments, vitals monitoring, safety checks).
- Develop, implement, and update care plans; collaborate with clients, families, providers, and internal care teams.
- Evaluate outcomes and adjust interventions to support client goals and independence.
- Educate and empower clients/families on condition management, medications, and safety.
- Coordinate and supervise caregiving staff within RN scope (as applicable), reinforcing best practices and Compassionate Care standards.
- Complete timely, accurate documentation per policy and Oklahoma regulations.
What It Takes
- Active Oklahoma RN/LPN license in good standing.
- 1 year of recent clinical experience (home health, hospice, primary care, or hospital preferred).
- Strong assessment skills, clinical judgment, and a calm, teach-first approach.
- Comfort working independently in the home and traveling locally (valid driver’s license, reliable transportation).

Schedule & Location
- Flexible full-time or part-time opportunities with primarily daytime visits (occasional flex as client needs evolve).
- Service area includes Eastern and South Central, OK.
