Job Overview
We are seeking an energetic and experienced RN Manager to lead the launch of our skilled home health care services. This full-time leadership role requires a dynamic professional with over three years of nursing experience who is passionate about building a high-performing team and establishing top-tier patient care programs.
Responsibilities
- Care Management
- Nursing Assessments
- Medication Management and Medication Administration
- Teaching and Training of Disease Processes
- Remote Patient Monitoring
- Nursing Delegation
- Makes the initial evaluation visit and regularly reevaluates the client’s needs
- Initiates the plan of care and necessary revisions.
- Provides services in accordance with the plan of care.
- Provides documentation of client assessments, plan of care, supervisory visits, caregiver and client education and training.
- Informs client and/or client representative of changes in the condition and needs of the client.
- Understands and adheres to established policies and procedures.
- Teaches and counsels the client and family/significant others regarding their service needs and other related problems of the client at home when applicable.
- Maintains accurate and complete records.
- Attends staff meetings, client care conferences and in-services as scheduled.
- Supervises and teaches other personnel, when applicable.
- All other duties as assigned.
Qualifications
- Graduate of a state approved school of nursing and currently licensed in the state in which practicing
- Maintains CPR Certification
- Minimum of three (3) years experience in Home Health or Hospice REQUIRED.
- Acceptance of philosophy and goals of Agency.
- Ability to exercise initiative and independent judgment.

Pay: $45.00 - $55.00 per hour
