As a Geriatric Care Manager, you will play a vital role in providing comprehensive care coordination for older adults. In this role, you will service as a central point of contact, coordinating medical appointments, overseeing medications, arranging in-home care services, and advocating for your client's best interests. A Geriatric Care Manager plays a vital role in enhancing the quality of life for older adults by providing holistic person-centered care.
Key responsibilities include:
· Needs Assessment: Conducting thorough evaluations to understand the unique needs and preferences of older adults, ensuring a tailored approach to care.
· Care Plan Development: Creating individualized care plans that address medical, social, and emotional needs, fostering overall well-being and dignity.
· Care Coordination: Efficiently managing appointments, medical services and interdisciplinary care teams, ensuring seamless continuity and consistency of care.
· Advocacy: Championing the rights of senior, ensuring they receive equitable access to the appropriate resources, care, and services.
· Resource Management: Identifying and facilitating access to valuable community resources, including home care agencies, transportation services, and social engagement programs, to enhance daily activities.
· Emotional Support: Offering compassionate emotional support and counseling to both seniors and their families, helping them navigate the complexities of aging and caregiving.

What We're Looking For
· Graduate of an Accredited Nursing Program with a valid RN Florida License.
· Minimum of 2 years Nursing/Care Management experience required
· Home Health experience required.
· Valid Florida Driver’s License and Automobile insurance.
· Strong goal oriented with the ability to think outside the box and find innovative solutions.
· Excellent written and verbal communication skills.
· Comfortable with personal computer use in a Windows and Excel environment .
Work Schedule:
· Monday through Friday, flexible hours to accommodate client needs.
· Occasional on Call duties on a rotational basis throughout the year. (minimal)
