The veterans we serve have often been failed by multiple systems before they arrive at Valor House. Many are navigating homelessness, serious mental illness, substance use disorders, chronic medical conditions, legal challenges, or all of the above.

WHY VALOR HOUSE?

Valor House serves veterans and former service members experiencing homelessness or housing instability. We are proud to operate Utah's only Grant and Per Diem (GPD) program serving female veterans while also providing services for male veterans through multiple housing and support programs.

As a Valor GPD Case Manager, you'll work alongside a dedicated multidisciplinary team helping veterans secure housing, increase income, access healthcare, connect with benefits, and move toward long-term independence.

Unlike many case management positions, you'll have a manageable caseload and the support needed to do meaningful work rather than simply checking boxes.

WHAT YOU'LL DO

  • Conduct assessments and identify barriers to housing and self-sufficiency.
  • Develop individualized service plans that reflect each veteran's goals and strengths.
  • Assist veterans with securing and maintaining permanent housing.
  • Support veterans in accessing VA benefits, Medicaid, SNAP, SSI/SSDI, and other resources.
  • Connect veterans with healthcare providers, behavioral health services, employment programs, legal resources, and community supports.
  • Collaborate closely with the Valor House team, VA partners, and community organizations.
  • Advocate for veterans when barriers arise.
  • Use Motivational Interviewing and trauma-informed approaches to foster engagement and recovery.
  • Provide life-skills coaching focused on budgeting, employment readiness, independent living, and community integration.
  • Complete accurate and timely documentation in HMIS and the electronic health record.
  • Participate in case conferences, program reviews, and quality improvement activities.

WHAT YOU BRING

Education & Licensure

  • Valid Utah Driver's License and insurability (required)
  • CPR/AED Certification (required)
  • Ability to complete annual suicide prevention training (required)
  • Ability to complete SOAR training after hire (required)
  • Utah Social Service Worker (SSW) license (preferred)
  • Case management experience (preferred)
  • Experience serving veterans (preferred)
  • Experience working with homelessness, serious mental illness, and/or substance use disorders (preferred)
  • Knowledge of housing programs, community resources, and public benefits (preferred)
