SUPERVISED BY: Director of Clinical Services
SUPERVISES: N/A
SUMMARY: Evaluates and designs plans to address and decrease barriers to medical, social, educational, and other needs essential to increasing self-sufficiency for clients.
DUTIES AND RESPONSIBILITIES- Works with the client to develop an Individual Case Management Plan.
- Provide case management services for clients as mutually agreed upon with the client in the Person-Centered Treatment Plan.
- Internal and external collaboration for client support and service needs, including coordinating transportation needs.
- Facilitate group treatment.
- Attend individual supervision sessions with the Director of Clinical Services, as directed.
- Attend all clinical meetings, with participation.
- Coordinates transportation needs of clients.
- Generate direct billable service hours according to First Step Home standards, as established in the annual review.
- Responsible for submitting all documents relevant to services rendered according to First Step Home, Inc. timelines.
- Works to attain the goals as written in job performance evaluations.
- Abide by the standards as set forth in First Step Home’s Compliance Plan and Code of Conduct.
- Pursue training necessary for enhancing clinical skills and acquiring and maintaining appropriate credentials and licenses.
- Responsible for following all safety rules and safe health practices of First Step Home, Inc.
- Comply with all agency policies, procedures, and directives of the immediate supervisor, the President/CEO, and the Board of Trustees of First Step Home.
- Responsible for following the Core Values of First Step Home, Inc.
- Other duties as assigned.
Physical Requirements:
- Prolonged periods of sitting at a desk and working on a computer.
- Must be able to lift up to 15 pounds at times.
QUALIFICATIONS
1. Bachelor’s degree in human services or related field preferred.
2. CDCA required.
3. Two years’ experience working in the field of behavioral health.
4. Current driver’s license in good standing and proof of personal auto liability insurance.
