Job Summary

Duties
- Provide comprehensive personal care assistance, including help with bathing, dressing, grooming, and mobility.
- Monitor patient health through observation of vital signs and assessment of overall condition.
- Assist with activities of daily living (ADLs) such as meal preparation, medication administration, and light housekeeping tasks like laundry and cleaning.
- Support clients with mobility transfers using patient transfer methods and equipment such as Hoyer lifts when necessary.
- Maintain accurate medical documentation and records using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems while adhering to HIPAA regulations to ensure patient confidentiality.
- Administer medications safely following medical terminology guidelines and home healthcare regulations.
- Provide specialized care for clients with dementia, Alzheimer’s disease, or in hospice settings, including memory care and end-of-life support.
- Assist with medical procedures such as catheter care and taking vital signs; respond promptly to emergencies with first aid or basic life support (BLS) and CPR when required.
- Communicate effectively with patients, families, and healthcare teams to coordinate care plans and update progress regularly.
- Support clients’ emotional well-being through compassionate companionship and engagement in meaningful activities.
Experience
- Prior experience working as a live-in caregiver or in home health care settings is highly preferred.
- Demonstrated experience assisting individuals with disabilities or chronic health conditions such as geriatrics or developmental disabilities.
- Knowledge of home health care assistance, hospice care, dementia or Alzheimer’s care, and memory care practices.
- Familiarity with medical terminology, medication management, medical documentation, and home health agency protocols.
- Hands-on experience using EMR/EHR systems for documentation; understanding of HIPAA compliance is essential.
- Experience working in assisted living facilities or nursing homes is advantageous.
- Ability to perform heavy lifting safely when required; familiarity with patient transfer methods like Hoyer lifts is beneficial.
- Certification in first aid, CPR, basic life support (BLS), or other relevant healthcare training is preferred but not mandatory.
