Job Summary:
The Registered Nurse (RN) is responsible for conducting initial client evaluations, developing Service Plans/Plans of Care, and performing supervisory visits in clients’ residences. This role ensures high-quality care, compliance with state regulations, and effective caregiver coordination. Responsibilities include assessing, documenting and implementing care plans, working collaboratively with care team, caregivers, clients, and families to ensure the best possible care for adult and geriatric patients

A Day in the Life of a FirstLight Home Care Registered Nurse:
Conducting initial and ongoing in-home assessments and caregiver introductions
Develop, implement, and adjust plans of care in compliance with state and agency standards
Fostering client relationships with each visit
Providing education and counseling to caregivers and family members regarding needs for services within FirstLight’s service boundaries
Participating in quality assurance reviews and evaluations of agency services
Providing limited skilled nursing services, where applicable, and within FirstLights’s service boundaries.
Maintaining accurate and timely documentation of services provided.
Who We’re Looking For:
A graduate of a state-approved nursing program
Possess an active Registered Nurse License in Kentucky.
At least one year experience preferably with home care, community nursing, and/or geriatric populations.
A passion for and commitment to ongoing professional development and adherence to best practices in nursing care.
Professional demeanor with empathy, patience, kindness, and respect
Proficient in Microsoft Office, Google Suite, and scheduling software.
Ability to pass a drug screen and background check
Valid driver’s license and auto insurance
Physical Demands & Work Environment
Frequent use of hands, fingers, and tools
Regularly required to communicate clearly and effectively
Occasional standing, walking, sitting, and reaching
Ability to lift and or more up to 50lbs.
Work is performed in both office settings and client homes with potential exposure to illness and bodily fluids.
