The Role
As an Outreach Case Manager, you’ll be on the front lines, connecting with unsheltered individuals and families to help them access housing, supportive services, and community resources. Using a trauma-informed care approach, you’ll build relationships with clients experiencing homelessness, mental health challenges, and substance use issues, guiding them toward stability and self-sufficiency.
You’ll collaborate with community partners, law enforcement, mental health providers, hospitals, and volunteers to identify the most vulnerable individuals and ensure they receive the care and support they need.
What You’ll Do
- Lead the Day Shelter for homeless clients, providing supervision, support, and engagement opportunities
- Conduct street and community outreach, engaging with unsheltered individuals and families
- Identify and assess housing barriers, mental health needs, and substance use challenges
- Develop individualized housing and service plans that promote stability and independence
- Connect clients to emergency shelters, permanent housing, benefits, and supportive services
- Maintain accurate client records, case files, and reports (including HMIS)
- Advocate for clients, helping them overcome barriers to housing, care, and social services
- Support volunteers in outreach ministry to expand the reach and impact
Key Performance Indicators (KPIs)
- Client Housing Placement Rate: Successful placement into emergency or permanent housing
- Client Engagement & Retention: Maintaining participation until housing is secured
- Case Management Efficiency: Time from initial assessment to housing placement
- Community Engagement: Building partnerships and positive relationships with service providers
What We’re Looking For
- Experience in outreach, case management, social work, or human services
- Knowledge of trauma-informed care, mental health, substance use recovery, and homelessness services
- Strong communication, advocacy, and relationship-building skills
- Ability to manage multiple cases with organization and professionalism
