Role Description
This is a full-time on-site Advanced Practice Provider role located in Jacksonville, FL. The Advanced Practice Provider will work collaboratively with a healthcare team to provide specialized nephrology care to patients. Responsibilities include conducting patient evaluations, developing and implementing treatment plans, managing acute and chronic conditions, coordinating care with other medical professionals, and providing patient education to support wellness and disease management.
Qualifications
- Strong foundation in Medicine and Primary Care
- State licensure
- Ability to work in a collaborative, multidisciplinary environment
- Excellent communication and interpersonal skills
- Flexibility to provide on-site care in Jacksonville, FL
- Background or experience in nephrology is a plus
- Master’s or Doctorate degree in Nursing or a related healthcare field
