Responsibilities
- Contact resident families upon admission and provide updates on resident’s progress.
- Contact referral sources and provide updates
- Assist client with issues such as FMLA and STD paperwork, food stamp applications, aftercare living and continued care arrangements
- Document all communication with outside agencies, family members and referents.
- Work with clients and treatment team to establish comprehensive discharge plans starting at time of admission.
- Establish relationships with community resources and alternative treatment programs.
- Keeps up to date information on community support resources.
- Makes appropriate referrals for clients when indicated.
- Serves on company committees, working groups, and other bodies as assigned
Qualifications
- Proven experience in a residential substance abuse or mental health treatment setting is required
- Professionalism and ability to relate to clients from all walks of life
- Familiarity with medical terminology, diagnostic evaluation procedures, discharge planning processes, and utilization management protocols.
Pay: $17.00 - $19.00 per hour
