Our Care Coordination team works directly with patients, their primary care physicians, specialist offices, and other participants in the patient’s healthcare team to assist patients with managing their healthcare needs. This job consists primarily of making outgoing phone calls to high-risk patients to provide patient education, support, and assist with coordination of services within and outside of our facility. The ideal candidate is someone who can work independently and has strong customer service skills. This position requires previous clinical experience (MA/LPN preferred).
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Provides coordinated care to (age-specific) patients by developing, monitoring, and evaluating interdisciplinary care
- Orients and educates patients and their parents by calling them; explaining the role of the patient care coordinator; initiating the care plan; providing educational information in conjunction with direct care providers related to treatments, procedures, medications, and continuing care requirements
- Evaluates outcomes of care with the interdisciplinary team by measuring intervention effectiveness with the team, implementing team recommendations.
- Responsible to review and follow up for Documents for all offices.
- Follow high risk patients, newborns and special requests
- Process documents in a timely matter using electronic health record (EHR) system.
- Follow and implement First Choice Pediatric Care Coordination Guidelines to allow for adequate clinical documentation.
- Review lab work, imaging, and other documentation for completion.
- Obtain any additional medical documentation needed prior to forwarding to MD
- Build and maintain relations within different FCP departments to maintain continuity of care
- Answer incoming calls from patients, employee, clients, & Physicians pertaining to Care Coordination
- Assist with all other duties of the department as needed
EDUCATION AND/OR EXPERIENCE: - LPN
- MA
- Med Tech
- Knowledge of medical terminology preferred
- Two years plus of customer service and medical office experience
- Electronic medical records experience, desirable.
- Clinical experience required.
