What are the responsibilities and job description for the Part-Time Home Health CNA position at First Choice Home Health & Hospice?
This position requires daily morning availability M-F. Roughly 15-20 hours per week.

Pay and Benefits:
Pay rate is from $19-25 an hour, DOE
First Choice pays travel time & mileage reimbursement
Responsibilities:
- Assists patients with their daily living activities
- Spends time assisting patients with grooming to help them feel great
- Works as part of the care team to help patients reach their goals and to be comforted
- Always maintains patient comfort & safety
- Attends case conference weekly to coordinate patient care with case manager & team
Qualifications:
Must be available daily Monday-Friday, beginning at 8 am.
Active CNA license *Please only apply if you have an active license.
Over the age of 18
Valid Driver's license
Reliable transportation
