Primary Care Licensed Practical Nurse (LPN)

First Choice Health Centers is looking for a full-time LPN to work in our outpatient primary care clinic. You will be responsible for performing various nursing tasks, such as taking vital signs, administering medications, collecting samples, documenting, and updating the patient electronic health record, calling patients back regarding their care plan and working with other facilities for referrals. You will also collaborate with other members of the health care team, such as physicians, nurse practitioners, and medical assistants, to ensure optimal patient outcomes.

• Primarily a Monday through Friday schedule working 8:00 am to 4:30 pm. There is one late night until 6 pm but you will get out on Friday at 3 pm. Rotating Saturday clinic from 8 am to 2 pm with a day off during the week when required.
• The hourly range for this position is $27.00 - $32.75/hour. Pay is based on several factors including but not limited to work experience certifications, etc.

Minimum Knowledge, Skills & Abilities Required:

• Experience with an electronic health record system
• Excellent communication and interpersonal skills
• A positive attitude and willingness to learn

Experience and Training:

• A valid LPN license in the state of Connecticut
• BLS Certification

Standard Job Duties:

• Provide direct and individualized nursing care to patients based on the application of the nursing process
• Maintains accurate medication inventory listings, oversees the CT Vaccine Program in accordance with Center policies and requirements including monthly inventory, ordering and documentation on each patient in the electronic health record with required elements
• Accurately documents in the electronic health record, all assessments, vital signs, specimen collections, interventions, evaluations and responses, teaching, procedures, narratives, referrals, etc. in the proper patient record

• Manages prescriptions and medication refills for providers in accordance with the Nurse Performs wound care using appropriate infection control practices
• Perform periodic meaningful use documentation including depression screening, sexual history, alcohol and substance abuse history and assessment. Refers appropriate patients to specialty providers in relation to the responses to the noted assessments
• Works with medical assistants to ensure safe and effective patient care is rendered in accordance with the medical assistant's scope of practice
• Interacts with outside agencies to manage patient care outside the Center

• Monitor task list (telephone encounters) in order to respond in a timely manner, follow up on overdue orders and monitor status of critical referrals
• Adhere to all patient safety standards and infection control guidelines
• Assist care team to assure smooth office operation and delivery of excellent service through teamwork.
