FCMC Behavioral Health Case Worker Job Requirements and Expectations for Employment:
- This is an in-person position.
- This role will primarily based at 404 S Sutherland Avenue, Monroe, NC 28112.
- The standard shift begins promptly at 9:00 AM ET. Arrival and clock-in at 9:00 AM ET constitutes on-time arrival.
- The shift is scheduled to end at 6:00 PM ET. However, all staff are required to remain on duty until the last patient has been treated and all necessary closing duties are completed.
- Schedules are determined by FCMC leadership based on clinic needs; FCMC is unable to accommodate requests for customized schedules (e.g., guaranteed days off, specific shift lengths).
- All staff are expected to abide by the First Care Medical Clinic uniform policy every day of employment, which includes matching scrubs and the lab coat or business attire.
- While staff are assigned to a primary location, all staff are part of a shared staffing pool and must be available for assignment to other FCMC clinic locations to ensure adequate patient care coverage, as directed by the Clinic Managers.
- Behavioral Health Case Workers are required to competently perform and maintain proficiency in all core case worker functions.
- Behavioral Health Case Workers are required to complete the training and onboarding as defined by First Care Medical Clinic.
- Chronic absenteeism or repeated tardiness will result in disciplinary action, up to and including termination.
- Violation of First Care Medical Clinic’s Code of Conduct or policies is grounds for immediate disciplinary action, up to and including termination.
- Failure to achieve or maintain the required level of competence for all core Case Worker functions is grounds for termination.
Key Responsibilities:
- Perform all job functions and patient engagement under the regulations of the Collaborative Care Model (CoCM).
- Engage patients using electronic, telephonic and/or video conference tools.
- Address patient referrals, appointments, and logistical needs.
- Document and track time in accordance with CoCM regulations.
- Conduct initial and ongoing assessments of clients’ behavioral health status, needs, strengths, and barriers.
- Gather information from various sources, including clients, families, healthcare providers, and records.
- Collaborate with patients and interdisciplinary teams to ensure care plans are realistic and achievable.
- Connect patients with appropriate behavioral health services (e.g., therapy, medication management, community resources).
- Facilitate referrals to housing, employment, education, and social support programs.
- Monitor the effectiveness of services and adjust care plans as needed.
- Help clients navigate barriers to accessing treatment and support.
- Provide education to patients and their family members about behavioral health conditions, treatment options, and self-care strategies.
- Offer emotional support and empower clients to participate actively in their recovery.
- Coordinate immediate interventions and support safety planning.
- Maintain accurate, timely records of assessments, care plans, interventions, and client progress.
- Comply with legal, ethical, and organizational standards for confidentiality and reporting.
- Work closely with healthcare providers, social workers, counselors, and community organizations to ensure continuity of care.
- Participate in team meetings and case conferences.
