Job Summary:
The Team Assistant plays a vital role in ensuring efficient care delivery and smooth daily operations. This position supports the interdisciplinary team by managing clinician schedules, maintaining accurate documentation, and facilitating clear communication among patients, clinicians, and staff.
Key Responsibilities:
- Coordinate and update clinician schedules to ensure full coverage and timely visits.
- Communicate effectively with clinicians, managers, and office staff via phone, email, and EMR.
- Assign patients to appropriate team members based on care needs and clinician availability.
- Adjust schedules as needed to meet patient timelines and regulatory requirements.
- Handle patient inquiries regarding start of care (SOC) and resumption of care (ROC) dates.
- Support on-call staff and coordinate PRN (as needed) visit coverage.
- Enter visit orders accurately and maintain up-to-date records.
- Assist with referral calls, data entry, and other administrative duties.
- Perform additional tasks as assigned to support team efficiency.
Qualifications:
- Compassionate, patient-focused, and dedicated to quality care.
- Experience working in a fast-paced medical or healthcare environment (required).
- Knowledge of Home Health operations (preferred).
- Strong organizational, multitasking, and problem-solving abilities.
- Excellent communication and customer service skills.
- Proficient in computer use, including EMR systems and Internet applications.
- High school diploma or GED (required).
