Essential Duties and Responsibilities
The position duties and responsibilities listed below describe the general nature and scope of work. Other responsibilities, duties and skills may be required and assigned, as needed.
§ Facilitate group and individual counseling sessions.
§ Maintain case files in described manner.
§ Complete intake, evaluation and referrals as needed for funding sources.
§ Write social histories, clinical assessments, and develop resident treatment plan(s).
§ Monitor resident progress toward goals and objectives as stated in the treatment plan and update the treatment plan(s) as required.
§ Serve as a member of the Classification/Case Conference Committee.
§ Write monthly progress reports on residents for the referring agency.
§ Supervise a caseload identified by the Clinical Director.
§ Serve as Facility liaison treatment services with referral agencies.
§ Make appropriate aftercare contacts for treatment services.
§ Performs other duties as assigned.
