The Finger Lakes Healthcare System has 1 vacancy for a Palliative Care Coordinator Registered Nurse (RN) at the Bath VA Medical Center. The RN is responsible and accountable for all elements of the nursing process when providing and/or supervising direct care to terminally ill and end of life patients. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.

Major duties include but are not limited to:
- Responsible and accountable for all elements of the nursing process when providing and/or supervising direct care to terminally ill and end of life patients.
- Assesses, plans, implements, and evaluates the delivery of palliative care services based on specific components.
- Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.
- Advocates and coordinates for specialized assessments and interventions to improve quality of life, symptom management, family support, and end of life issues.
- Follows procedures per established policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team to include working with outside agencies and resources in the community.
- Core elements of performance include knowledge and active participation in the unit/program level quality improvement process and initiatives as well as customer service programs.
