Fifth Street Ministries is seeking a Safe at Home Housing Case Manager to help individuals and families move from homelessness into safe, stable housing—and thrive there.
In this role, you’ll:
✨ Provide person-centered case management under the Housing First model
✨ Meet monthly in person with clients to review progress and set attainable goals
✨ Connect clients to resources like healthcare, job training, childcare, and benefits
✨ Coordinate or lead group sessions on budgeting, parenting, life skills, and more
✨ Advocate for client needs with community partners and service providers
✨ Maintain accurate case notes, reports, and other documentation
✨ Collaborate with a supportive team and network with outside agencies
Education & Experience:
🎓 Associate degree in a related field and at least two years of relevant work experience OR five consecutive, recent years in a similar role
🗣 Strong written communication skills with the ability to produce clear, professional, and grammatically correct documents
👂 Excellent listening and verbal communication skills
