Position Summary
The Home Health Aide (HHA) provides direct, non-medical care and assistance to clients in their homes under admin The HHA supports clients with personal care, activities of daily living, and basic health-related services in accordance with the individualized care plan, ensuring safety, comfort, and dignity at all times.

Key Responsibilities
- Assist clients with activities of daily living (ADLs), including:
- Bathing, grooming, and dressing
- Toileting and continence care
- Mobility, transfers, and safe ambulation
- Feeding and meal preparation
- Provide companionship and emotional support.
- Perform light housekeeping and laundry to maintain a safe, clean home environment.
- Remind and assist with medication (in accordance with state regulations and agency policies).
- Take and record vital signs (temperature, pulse, respiration, blood pressure) as instructed.
- Observe, report, and document changes in the client’s condition to the supervisor.
- Assist with exercises or rehabilitation activities as directed.
- Maintain confidentiality and adhere to HIPAA and agency policies.

Qualifications
- High school diploma or equivalent preferred.
- Must hold a valid Home Health Aide (HHA) certification from a state-approved training program.
- Current CPR and First Aid certification preferred.
- Ability to read, write, and follow care plan instructions.
- Compassionate, reliable, and patient with strong interpersonal skills.
- Physical ability to lift, move, or assist clients as needed.

- May require occasional travel between client locations.
- Work involves standing, walking, bending, lifting, and assisting with client mobility.
