Our company is growing and we are looking for a dynamic RN, BSN with a Master’s degree, preferably in nursing or MSW, related social service fields will be considered. Nurse Practitioners need not apply. Come grow with us!
- Case Management experience a plus.
- Must live within 20 miles of The Villages.
- Must have community experience and knowledge of community resources.
- Marketing experience is a plus.
- Flexibility is a must.
- Weekend on call once a month is required.
- Travel in Central Florida is required. Must have reliable transportation.
- This is a part time including travel
- Ability to work 20 hours /week
- Pay will be discussed upon interview
Overview of Responsibilities:
- Conducting Assessments and developing/implementing individualizes care plans
- Coordinating care and services
- Monitoring progress by conducting regular site visits and accompanying clients to medical appointments
- Educating and supporting clients and their families
- Documentation and maintaining records
Pay: $30.00 - $40.00 per hour
