Job Summary
We are seeking a dedicated and energetic leader to lead the Assisted Living and Memory Care Floors. This person must have an active CNA (Certified Nursing Assistant), TMA (Transport Medical Assistant), or LPN (Licensed Practical Nurse). In this vital leadership role, you will oversee daily patient care operations on the floor, ensuring that high-quality, compassionate care is delivered efficiently and safely. Your expertise will guide and support frontline staff, coordinate patient services, and uphold compliance with healthcare standards. Responsibilities
- Lead and supervise nursing staff, including CNAs, TMAs, and LPNs, to ensure seamless delivery of patient care across the unit
- Coordinate patient admissions, discharges, and transfers while maintaining accurate medical documentation in EMR (Electronic Medical Records) or EHR (Electronic Health Records) systems
- Oversee medication administration, vital signs monitoring, catheter care, and other clinical procedures following aseptic techniques and safety protocols
- Assist with complex patient needs such as Alzheimer's care, memory care, dementia care, and behavioral health management
- Ensure compliance with HIPAA regulations and maintain confidentiality of all patient information
- Support staffing
Requirements
- Valid CNA certification, TMA certification, or LPN license in the state of practice
- Prior experience working in diverse healthcare settings including hospitals, long-term care facilities, assisted living communities, or home & community care environments
- Knowledge of EMR/EHR systems
- Strong understanding of medical terminology, anatomy knowledge, and physiology principles relevant to inpatient and outpatient care
- Experience with specialized patient populations including individuals with developmental disabilities or those requiring hospice or infectious disease care
- Ability to lead a team effectively while maintaining a positive attitude and fostering a culture of safety and excellence
